All community service hours and donations must be submitted to the UPHA office by October 31. Please submit a Community Service hour log monthly.
Community Service is defined as “that a person performs for the benefit of his or her local community on an unpaid, volunteer basis.” Please refer to Community Service Guide for eligible and ineligible hours. UPHA reserves the right to disqualify any hour(s) of community service deemed improper, inappropriate or inconsistent with the purpose of the program as defined in the rules.
